Payment
Payment of the package fee or consultation (for follow ups booked in addition to a package) is required upon booking by BACS bank transfer and a Nutritional Therapy Questionnaire and food and lifestyle diary will be emailed to you within 24 hours. If you would prefer a hard copy please let me know when booking.
Paperwork
Your Nutritional Therapy Questionnaire should be completed and returned to me at least 72 hours prior to your consultation to allow me time to consider your medical history, medication/supplement interactions and current diet and lifestyle. Everything sent to me is treated confidentially.
Cancellation
If you need to cancel or rearrange an appointment, please email me at least 48 hours prior to your appointment.
Refunds
Please note that the full consultation fee will be charged if you fail to keep your appointment or cancel/change it with less than 48 hours notice (changes for consultations on a Monday must be made the previous Friday). In the event that I am able to re-book your appointment, there will be no charge. Once paperwork has been received for a consultation, no refunds will be given.
It is your responsibility to have a working internet connection for all online consultations. You will be charged for a scheduled consultation if you are unable to connect online. I will make every effort to contact you in the event that you are unable to join an online consultation and offer to switch to a telephone call in the event of technical difficulties your part.
Testing
Any functional laboratory tests and supplements are charged in addition to the package or consultation fee. Where appropriate and possible, clients will be referred to their GP for NHS testing.
Confidentiality
Clients of Jo Santini are confidential as well as all information shared with Jo Santini and all discussions within clinic consultations. There are however, some exceptions to confidentiality. If I have serious concerns about your personal safety, or the safety of another person normal confidentiality may be lifted.
